Transfer of Local Church Membership
Please complete this form if your church is leaving the United Methodist Church, but you want to remain a member of the United Methodist Church.

There are two options:
- Designate a United Methodist Church to place your membership or
- Place membership temporarily with the general membership role of the Horizon Texas Annual Conference
Either option will allow you time to explore other United Methodist churches in your area.

It is important to note that all lay members of The United Methodist Church who serve in District, Conference, or General Church leadership roles must maintain unbroken membership in The United Methodist Church to continue in those positions.

To transfer your membership and remain a member of The United Methodist Church, simply complete the information fields below. Communication will be sent to both the releasing and receiving churches (or the Annual Conference) to honor the desires of those who wish to remain United Methodist.
